Since apparently people enjoy the USCSB Youtube channel, I thought I'd post IMHO their most interesting video: In a twist of fate, multiple errors in the manufacture of the Blowout Preventer cancelled each other out, meaning the Blowout Preventer was partially functional. Critical safety errors cancelling each other out is something I've never seen in any disaster post-mortem.
